Duo access is attached to GitLab plans and credits, so cancel the containing GitLab subscription.

Open the Customers Portal
Sign in at customers.gitlab.com and open Subscriptions & purchases.

Find the subscription that provides Duo
Locate the Premium, Ultimate or other GitLab subscription linked to the namespace and confirm its renewal date.

Choose Cancel subscription
Open Subscription actions on that card and select Cancel subscription.

Confirm non-renewal
Select a reason, optionally add feedback and confirm. Save proof that the card now shows an expiry date instead of auto-renewal.
Submit the cancellation at least 30 days before the term expires.
Order forms and committed GitLab Credits can have additional terms.
